初めて外来を受診される方(初診)
受付にて健康保険証（マイナンバーカード）をご提示ください。マイナンバーカードの電子証明書の有効期限にご注意ください。スマートフォンに搭載されたマイナンバーカードでの受診も可能です。
受付にてお渡しする「外来診療における同意書」の説明事項をお読みいただき、必要事項をご記入ください。
紹介状をお持ちの方は受付でお渡しください。
六本木ヒルズクリニックにて一般外来または人間ドック・健康診断を受診されたことがある方は、診療情報を全て引き継いでおりますのでお申し出ください。磁気カードの診察券も表面のバーコードをそのままご利用いただけますのでお持ちください。新たな磁気カードの診察券は発行しておりません。予約票やスマホアプリのバーコードをご利用ください。
First-visit patients (New patients)
Please present your health insurance card (My Number Card) at the reception. Also, please check the expiration date of digital certification of My Number Card beforehand. My Number Card (as health insurance certificate) installed on a smartphone is accepted.
You will be given a consent form for outpatient consultation at the reception. Please read it through and fill in some items required.
Please present a referral letter at the reception if any.
Please make sure to inform us if you have ever visited the outpatient department or undergone the Ningen Dock/health checkup at Roppongi Hills Clinic because your medical information has been fully carried over to Plaza Clinic. If you have the magnetic clinic ID card, please bring it as the bar code on it remains effective. However, we do not newly issue a clinic card anymore. Please use the bar code printed on the appointment slip or the one displayed on the smartphone app.

2度目以降の方（再診）
自動再来受付機に予約票、診察券（旧六本木ヒルズクリニック）、スマホアプリのバーコードをかざしていただき、受診される科をお選びください。
受診票がA4とレシート1枚ずつ印刷されますのでA4の受診票のみファイルに入れ処置室前のボックスへお出しください。レシートは会計終了までお持ちください。
自動再来受付機に設置のマイナンバーカード読み取り機にてマイナ保険証の読み取りを行っております。マイナンバーカードの電子証明書の有効期限にご注意ください。
スマートフォンに搭載されたマイナンバーカードでの受診も可能です。
Revisit-patients (returning patients)
Hold your appointment slip, clinic ID card (under the name of Roppongi Hills Clinic) or bar code displayed on the app over the reader of the self-check-in machine for revisiting, then choose the department of your consultation. A reception sheet (A4 size) and its receipt (a small slip) are printed for you. Put A4 sheet in the plastic folder and place in the box in front of the treatment room. Please keep the small slip (receipt) until the payment.
My Number Card reader is set at the self-check-in machine. Please hold your My Number Card over it. Please check the expiration date of digital certification beforehand. My Number Card (as health insurance certificate) installed on a smartphone is accepted.

当院では、医療DXを推進し、質の高い診療を実施するため、下記の取り組みを行っております。

- オンライン資格確認を行う体制を有しています。
- オンライン資格確認を利用して取得した診療情報を、閲覧または活用できる体制を有しています。

Notice Regarding Add-on for System Establishment of the Medical DX (digital transformation)
Our clinic is implementing the following initiatives to advance medical DX and deliver high-quality care.
- We accept online billing.
- We have a system in place to perform online verification of qualification.
- We have a system in place to view or utilize medical information obtained through online verification of qualification.
- We are establishing an environment that facilitates the use of My Number Cards as health insurance cards.
- We will prepare a system capable of issuing electronic prescriptions and utilizing electronic medical record information sharing services.

スマートフォンの医療情報管理アプリ「NOBORI」のご紹介
当院ではスマートフォンで医療情報管理アプリ「NOBORI」をインストールし、連携することで下記のサービスを利用できます。
- 通院履歴や予約を確認できます。予約票として自動再来受付機が利用できます。
- 診察の順番を表示できます。受付後外出されても順番が近づきましたらお呼びします。
- 血液・尿検査、画像検査の結果を表示できます。
- あらかじめクレジットカードを登録することで医療費の後払いができます。診察後は会計を待たずにご帰宅いただけます。診察前に受付にて「後払い利用が確認できる画面」をご提示ください。
- 健診センターで受診された人間ドック、健康診断の結果を表示できます。

Introduction of NOBORI – Smartphone app for medical information management
The following services are available for you at our clinic by installing the medical information management app "NOBORI" to your smartphone.
- Check your next appointment as well as the past visit history. The app works as an appointment slip which enables you to use the self-check-in machine.
- Check the queue status.
- View your examination results such as blood test, urine test, and imaging tests.
- Choose pay later to skip the payment procedure in the clinic after your consultation by registering your credit card information beforehand.

未成年者[18歳以下の方]の外来受診について
〈未成年者の受診における当院の方針〉 未成年者[18歳以下の方]が受診する場合、原則「親権者の同伴」が必要です。同伴を必要とする理由は以下の通りです。
- 症状・既往歴・内服薬・アレルギー等、診療に必要な情報を正確に把握するため。
- リスクを伴う検査や処置、使用する薬剤について適切にご理解いただくため。
- 検査結果や診療の方向性についての説明を正確にご理解いただくため。
やむを得ず親権者がご同伴いただけない場合、親権者に「同伴同意書」を記載いただくことにより、代理者による同伴受診を受付いたします。
受診日当日にお持ちいただくもの：
- 未成年者の診察券（旧六本木ヒルズクリニックのものをお持ちの方）・保険証（マイナンバーカード）・医療証
- 親権者により記載された同伴同意書
- 親権者の写真付身分証明書コピー
- 同伴者（受任者）の写真付身分証明書 ※当院保管のため一部コピーをさせていただきます。
Patients underage (under 18 years old) visiting the outpatient department
- To correctly understand medical information required for consultation such as symptoms, past medical history, medications, allergies and so on.
- To have your appropriate understanding on possible risks associated with examinations and treatments as well as medicines used.
- To have your correct understanding on examination results and direction of treatment.
If the parent or guardian is unable to accompany the minor, we will accept a proxy accompanying the minor for consultation based on the condition that the parent or guardian fills out a "Consent Form for Accompanying a Minor."
Items required to present at the reception on the day of consultation:
- Clinic ID card of a minor issued by Roppongi Hills Clinic (if any), health insurance card (My Number Card), medical care certificate for a child
- Consent form for accompanying a minor
- A copy of photo ID of a parent or guardian
- Photo ID of a proxy ※We take a copy for our file.
